Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and the plan of care, coordinating with the health care team.
• Documents patient visits thoroughly in the electronic medical record per regulatory and payer requirements.
• Participates in care coordination, communicating with patient, family, physician, clinicians, and case coordinators.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
