Job summary

Suncrest Home Health, part of the LHC Group and within the Optum family, is hiring a Licensed Practical Nurse to provide quality in-home patient care. This role operates in patients' homes, coordinating with care teams and following the plan of care while adhering to infection control requirements.

Applicants must have an active LPN license, a valid driver’s license or reliable transportation, and be able to work flexible hours. CPR is preferred; home care experience is a plus.

Qualifications

• LPN licensure in state of practice is required.
• CPR certification is preferred or to be obtained within 90 days of hire.

Responsibilities

• Provides high quality clinical services within the scope of practice per plan of care and infection control procedures.
• Documents patient visits in EMR and visits verification system.
• Participates in care coordination and communicates with patient, family, physician and care team.
• Ability to work in diverse home settings and serve clients regardless of background.
