Job summary

CHRISTUS HomeCare, part of LHC Group and within the Optum family, is hiring a Licensed Practical Nurse to provide quality in-home health services. The role emphasizes patient-centered care, coordination with the health care team, and adherence to infection control and regulatory requirements.

You will document visits, participate in care planning, and deliver compassionate support to patients and families in their homes.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and access to a dependable vehicle or reliable public transportation.

Responsibilities

• Provide high quality clinical services within the scope of practice per plan of care.
• Document patient visits thoroughly in the EMR per regulatory and payer requirements.
• Participate in care coordination and communicate with patients, families, physicians and team members.
• Function effectively in home environments regardless of patient demographics.
