Lutheran Senior Services is seeking a Licensed Practical Nurse (LPN) to provide and coordinate care for residents in our Peoria, IL location. The role requires active LPN licensure in the state, CPR certification, and the ability to work within state/national regulations while following facility policies.

Qualifications

• LPN licensure in state of practice.
• Must be CPR certified.
• Specialized training or experience in gerontological nursing preferred.

Responsibilities

• Practices nursing care within licensure and applicable laws.
• Collaborates with interdisciplinary team to plan individualized care.
• Administers medications and monitors IV fluids and treatments.
• May initiate IV therapy if IV-certified.
• Maintains documentation and care plans in the EMR system.
• Ensures safety and a clean, functional environment for residents.
• Observes resident status and coordinates with services for continuity of care.
• Oversees staff to ensure standard of care is met.
• Reports incidents per policy and regulatory guidelines.
