Job summary

CCH Healthcare in Five Forks, OH is seeking licensed practical nurses (LPNs) to join our skilled nursing team and provide compassionate, resident-centered care. You will administer medications, monitor residents, and collaborate with physicians, families, CNAs, and the broader care team in a supportive, team-focused environment.

Qualifications

• Current and valid LPN license.
• Long-term care or skilled nursing experience preferred, but not required.
• Strong clinical, communication, and organizational skills.
• Compassionate and resident-focused approach to care.
• Ability to work effectively as part of a nursing team.

Responsibilities

• Provide direct nursing care to residents in accordance with their care plans.
• Administer medications and treatments as ordered.
• Monitor residents and promptly report changes in condition.
• Complete accurate and timely nursing documentation.
• Communicate with physicians, families, CNAs, and other members of the interdisciplinary team.
• Supervise and support CNAs during assigned shifts.
• Follow infection-control, safety, and facility policies.
• Help maintain a comfortable, respectful environment for residents.
