Job summary

StoneBridge Senior Living is seeking a Licensed Practical Nurse (LPN) to join our dedicated team. You will coordinate nursing care for residents, administer medications, and provide treatments per physician orders in a compassionate, elder-focused environment.

Requires an active LPN license and CPR certification; 2+ years in long-term care preferred. Proficiency with Electronic Medical Records is encouraged to support resident care.

Qualifications

• Must have a current state license to practice as an LPN and CPR certification; additional nursing certifications are a plus.
• Demonstrates knowledge of basic nursing and gerontological care.
• Prefer 2 years of experience in a long-term care setting.
• Basic computer skills and experience with Electronic Medical Records are preferred.

Responsibilities

• Coordinate nursing care for residents on the unit with assigned nurse aides.
• Administer and document medications and treatments per schedule.
• Provide treatments per physician orders (wound care, catheterization, oxygen, tracheostomy care).
• Notify physicians of status changes and implement new orders promptly.
