Job summary

StoneBridge Senior Living is seeking a licensed practical nurse (LPN) to join our care team. You will coordinate nursing care for residents, administer medications, and document treatments in an EMR system with a focus on geriatric care.

We value compassion, teamwork, and reliable schedules. The role involves direct patient care in a long-term care setting and requires CPR certification and an active LPN license.

Qualifications

• Must hold a current state license to practice as an LPN and CPR certification.
• Demonstrates knowledge of basic nursing practice with an interest in gerontological nursing.
• Prefer 2 years of experience in a long-term care setting.
• Basic computer skills and experience with EMR are preferred.

Responsibilities

• Coordinates the nursing care of residents on the unit and ensures each resident has appropriate nurse aide support.
• Uses independent judgment in performing duties on behalf of the organization.
• Provides safe and accurate medication-related interventions and documents schedules.
• Administers and documents medications and treatments according to each resident’s schedule.
• Performs nursing assessments and notifies physicians of status changes; implements new orders.
• Prepares assignment sheets considering resident safety and staff capabilities.
• Reports staff shortages to nursing management and documents actions.
