Job summary
Senior Community Care of North Carolina Home Health is seeking an LPN for the Med Room at the PACE clinic. Schedule: Monday–Friday 8:30 AM–5:00 PM with on-call rotation.
On-site position with a strong benefits package including 403(b), medical/dental/vision, paid time off, and continuous education. The LPN will administer medications, assist with care plans, and collaborate with an interdisciplinary team in clinics and participant homes.
Qualifications
- Graduate of an accredited school of nursing.
- Current Licensed Practical Nurse (LPN) license.
- CPR certification required.
- Minimum of one year’s experience with frail/elderly population.
- Must have valid driver’s license, proof of insurance and transportation.
- Medical clearance for communicable diseases and up-to-date immunizations.
- Completes basic assessments and assists RNs as delegated.
- Functions as a member of the Interdisciplinary Team with 24-hour care delivery.
Responsibilities
- Administers medications and treatments per plan.
- Performs nursing care in the clinic and homes.
- Evaluates participant conditions and communicates changes to the care team.
- Provides education to participants and caregivers on plan of care.
