The Licensed Practical Nurse (LPN) at Trilogy Health Services in Goshen, IN provides direct clinical care on the assigned unit and supervises nursing assistants. Qualifications

• Must have and maintain a current, valid state LPN license.
• Current CPR certification required.

Responsibilities

• Visit residents daily to assess health status and alert care team.
• Identify problems and initiate lifesaving measures in absence of physician.
• Provide holistic nursing care within LPN scope.
• Monitor medications, supplies, and equipment levels.
• Complete medical forms, reports, and charting as needed.
• Report exposures per facility policy.
