Job summary

AccessStar is seeking a Licensed Practical Nurse (LPN) in Culpeper, VA to provide compassionate nursing care for individuals with IDD across multiple Virginia locations. You will administer medications, monitor health status, assist with daily activities, and collaborate with a multidisciplinary team to develop and update care plans.

The role emphasizes documentation, safety, and adherence to regulations, while promoting residents’ dignity and independence within AccessStar policies.

Qualifications

• Valid LPN license in the state of Virginia.
• Experience administering medications and treatments, preferably with individuals who have IDD.
• Strong clinical assessment, critical thinking, and problem-solving skills.
• Strong documentation and assessment skills.
• Ability to maintain confidentiality and uphold professional standards of care.
• Excellent communication and interpersonal abilities.
• Ability to work effectively as part of a team and contribute to team goals.
• Commitment to promoting the dignity and independence of residents.
• Knowledge of safety procedures and infection control techniques.

Responsibilities

• Direct Patient Care: administer medications and treatments per guidelines and Six Rights of Medication Pass; monitor vital signs and condition changes; assist with activities of daily living; provide wound and catheter care.
• Health Monitoring and Documentation: maintain accurate patient records; ensure compliance with health and safety standards; document medication administration and refusals.
• Collaboration and Communication: work with physicians, RNs, DSPs, and therapists; participate in care planning; report concerns to Nursing Manager.
• Health Promotion and Advocacy: support residents' health goals and promote healthy lifestyle choices.
• Training and Support: train DSPs on healthcare tasks; assist in health education programs for IDD individuals.
• Safety and Compliance: use PPE and infection control techniques; report accidents or hazards; ensure safe storage of oxygen and proper inventory handling.
• Supervision and Meetings: may supervise DSPs; attend regular meetings and trainings.
• Additional Responsibilities: maintain required trainings and certifications; perform other duties as assigned.
