Caregiver is seeking a compassionate LPN/LVN to join our team in Carrollton, TX, to support individuals with IDD. You will work under RN guidance to provide routine nursing care, monitor health, and administer medications in a safe, person-centered environment.

Qualifications

• Active LPN/LVN license (multi-state preferred).
• Graduate of an accredited LPN/LVN program.
• 1+ year of nursing experience preferred.
• Experience with IDD, developmental disabilities, or long-term care preferred.
• Familiarity with EHR systems and basic computer tools.

Responsibilities

• Provide routine nursing care and health monitoring.
• Administer medications and treatments as directed.
• Observe and report changes in health or behavior.
• Assist with daily living activities (ADLs) like hygiene, mobility, and nutrition.
• Support implementation of individualized care and behavior plans.
• Maintain accurate documentation and medication records.
• Ensure proper medication storage, handling, and inventory.
• Respond to emergencies and provide immediate care when needed.
• Collaborate with RNs, providers, and interdisciplinary teams.
