National Health Care Associates seeks a dedicated Licensed Practical Nurse (LPN) for a full-time evening shift (3p-11p) at Village Crest Center for Health & Rehabilitation in New Milford, CT. You will provide direct nursing care, administer medications, and support care plans alongside a multidisciplinary team.

Qualifications

Valid state LPN license.
Compassionate and empathetic approach to patient care.
Interest in nursing needs of the aged and chronically ill.
Strong clinical skills including administration of medications.
Excellent communication and interpersonal abilities.
Ability to work collaboratively with a multidisciplinary team.

Responsibilities

Administer medications and treatments as prescribed by healthcare providers.
Monitor and record patient vital signs and medical information.
Collaborate with the interdisciplinary team to develop and implement care plans.
Provide wound care and other nursing interventions as needed.
Supervise CNAs in delivering care while ensuring quality and safety.
