All American Assisted Living in New Jersey seeks an LPN to join our Wellness team. You will provide compassionate nursing support, administer medications, monitor residents, maintain documentation, and collaborate with residents, families, providers, and the care team.

Qualifications

• Active New Jersey LPN license
• License in good standing
• Knowledge of nursing standards and medication administration
• Observation and resident-monitoring skills
• Ability to work under RN direction
• Strong documentation and time management
• Experience with electronic health records
• English communication skills
• Experience with older adults preferred

Responsibilities

• Administer medications and treatments per orders and procedures
• Monitor residents for changes in condition
• Record vital signs and health information
• Provide direct nursing care within the NJ LPN scope
• Document and communicate resident information to RN or nurse leader
• Report significant changes promptly
• Coordinate medication refills with pharmacies
• Maintain accurate medication and treatment documentation
• Support admissions and transitions
