Job summary

Blue Ridge Hospice seeks a dedicated RN or LPN for a part-time in-home nursing role, delivering direct clinical care to older adults in their homes, ALFs, and community settings.

You will perform assessments, medication management, wound care, and chronic disease monitoring, coordinating with the interdisciplinary team to support safety, independence, and dignity.

This schedule offers Friday through Sunday coverage, 16–30 hours weekly, with differential pay and ongoing professional development.

Qualifications

  • Current RN or LPN licensure in Virginia.
  • Current BLS/CPR certification.
  • Minimum 1 year experience with frail/elderly population.
  • Strong clinical assessment and critical thinking skills.
  • Experience with home health, geriatrics, hospice, or community-based care preferred.
  • Medical clearance for communicable diseases and up-to-date immunizations.
  • Valid driver’s license and reliable transportation.
  • Ability to work independently while maintaining team communication.

Responsibilities

  • Conduct in-home nursing assessments and ongoing evaluations.
  • Perform skilled visits: med management, wound care, injections, phlebotomy, chronic disease monitoring.
  • Assess condition changes and communicate with the IDT.
  • Develop and monitor individualized care plans with IDT.
  • Educate participants and caregivers on disease management and safety.
  • Coordinate post-hospitalization transitions and care settings.
  • Support participants in ALF or community settings as needed.
  • Identify home safety concerns and address environmental risks with PT/OT and social work.
  • Participate in IDT meetings and holistic care planning.
  • Document nursing care accurately in the EMR.
  • Respond to urgent needs and participate in on-call rotation.
  • Monitor medication adherence and assist with reconciliation.
  • Collaborate with external providers and home health agencies.
  • Perform other related duties as assigned.

Licensed Practical Nurse (LPN) / Registered Nurse (RN) – PACE (Part-Time)
About the Role

We’re seeking a dedicated Registered Nurse (RN)to join our PACE interdisciplinary team in a primarily field-based, in-home nursing role. In this position, you will provide direct clinical care to participants in their homes, assisted living facilities, and community settings, supporting older adults in maintaining safety, independence, and dignity within their preferred living environment.

This role is ideal for a nurse who values autonomy, strong clinical assessment skills, and meaningful relationships with participants and families.

Schedule
  • Part-Time Position

  • Friday through Sunday schedule

  • 16-30 hours per week

  • Eligible for applicable shift differentials

How You'll Make a Difference
  • Conduct comprehensive in-home nursing assessments and ongoing evaluations of participant status.

  • Perform skilled nursing visits including medication management, wound care, injections, phlebotomy, and chronic disease monitoring.

  • Assess changes in condition and communicate promptly with providers and the interdisciplinary team (IDT).

  • Develop, implement, and monitor individualized care plans in collaboration with the IDT.

  • Provide participant and caregiver education on disease management, medications, safety, and prevention strategies.

  • Coordinate care transitions following hospitalizations, ER visits, or facility stays.

  • Support participants residing in ALF or other community settings as needed.

  • Identify safety concerns in the home and collaborate with PT/OT and social work to address environmental risks.

  • Participate in interdisciplinary team meetings and contribute to holistic care planning.

  • Document all nursing care accurately and timely in the electronic medical record.

  • Respond to urgent clinical needs and participate in on-call rotation as assigned.

  • Monitor medication adherence and assist with medication setup or reconciliation.

  • Collaborate with external providers, pharmacies, home health agencies, and specialty services.

  • Perform other related duties as assigned.

What You Bring
  • Current Registered Nurse (RN) or LPN licensure in the Commonwealth of Virginia.

  • Current BLS/CPR certification.

  • Minimum of one year of experience working with the frail or elderly population.

  • Strong clinical assessment and critical thinking skills.

  • Experience with home health, geriatrics, hospice, or community-based care preferred.

  • Medical clearance for communicable diseases and up-to-date immunizations prior to participant contact.

  • Valid driver’s license, proof of insurance, and reliable transportation (required due to field-based nature of the role).

  • Ability to work independently while maintaining strong communication with a collaborative team.
