Job summary

Bethesda Health Group seeks a dedicated Licensed Practical Nurse to provide basic nursing care and participate in care planning for geriatric residents. You will assess changes in condition, document interventions, and communicate with the IDT and families.

The role includes medication administration, IV work if certified, and using SBARR for physician communication. Strong clinical judgment, clear communication, and commitment to ongoing education are essential.

Qualifications

• Licensed Practical Nurse in the state you will be working as.
• IV Certification required or willing to obtain within six months of employment.
• CPR/AED Certification required or willing to obtain within six months of employment.
• Must be able to assess the basic nursing care needs of the geriatric resident.
• Excellent communication skills to interact with residents, visitors and staff.
• Must possess critical thinking skills.

Responsibilities

• Provides basic nursing care and assesses residents on admission and ongoing.
• Documents assessments accurately and communicates changes to the IDT and families.
• Implements nursing interventions, including orders and education for residents and families.
• Ensures safe medication administration and treatments; administers IV fluids if IV certified.
• Notifies physician promptly using SBARR; responds calmly to crises and maintains composure.
• Observes residents closely to ensure care plan delivery and engages in QAPI-driven problem resolution.
• Answers call lights and assists with ADLs; participates in continuing nursing education.
