Job summary

The Willow Terrace Nursing and Rehabilitation Center in Philadelphia, Pennsylvania is seeking a compassionate Licensed Practical Nurse (LPN) to join our care team. This full-time role focuses on delivering high-quality resident care, coordinating with a multidisciplinary team, and supporting families with education and care planning.

Qualifications include current PA LPN licensure and preferred prior experience.

Qualifications

• Current LPN licensure in Pennsylvania.
• Prior experience preferred.
• Strong communication and interpersonal skills.
• Ability to work effectively in a fast-paced environment with multiple priorities.

Responsibilities

• Provide 24-hour care to residents, including vital sign checks, medication administration, and wound care management.
• Conduct assessments and observations to identify changes in resident condition and report to RNs and other healthcare professionals.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans.
• Offer health education to residents and family members, and facilitate communication with healthcare teams.
• Provide complex medical care including tracheostomy care, IV therapy, enteral/parental feedings, wound care, and pain management.
• Liaise with healthcare teams to ensure seamless care coordination.
• Support a wide range of health concerns such as hospice, cardiac, podiatry, physiatry, psychiatry and psychology services.
