Qualifications

• Must be a Licensed Practical Nurse (LPN) with a current license to practice in Pennsylvania (PA).
• Prior experience in a healthcare setting.

• Excellent communication and interpersonal skills, with the ability to work collaboratively with the interdisciplinary care team.

Responsibilities

• Monitor and report changes in resident condition and address concerns.
• Educate residents, families, and caregivers on care instructions.
