Job summary

Countryside Post-Acute and Rehabilitation Center is seeking a Licensed Practical Nurse (LPN) to deliver compassionate care to residents in a post-acute and long-term setting. You will administer medications, monitor vitals, perform wound care, and help implement individualized care plans with the healthcare team.

The role requires current LPN licensure and 0-2 years in post-acute care; strong communication and teamwork are essential.

Qualifications

• Current LPN licensure.
• 0-2 years of experience in a post-acute care setting (new graduates welcome to apply).
• Strong communication and interpersonal skills.
• Ability to work collaboratively as part of a team.
• Basic computer skills and familiarity with electronic medical records.

Responsibilities

• Provide direct patient care to residents, including administering medications, wound care, and vital sign monitoring.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Document resident progress, report any changes or concerns to the healthcare team, and participate in interdisciplinary care conferences.
• Collaborate with other healthcare professionals to ensure seamless transitions and optimal outcomes.
• Maintain accurate and up-to-date medical records.
