You will manage the client intake process from inquiry through enrollment, conduct screenings and interviews, and maintain accurate records while collaborating with care teams to ensure smooth service placement.

Qualifications

• Experience in intake coordination or related social services fields.
• Strong interpersonal and communication skills with a client-first approach.
• Proficiency in data entry and using electronic health records (EHR) systems.
• Ability to multitask and stay organized in a fast-paced environment.

Responsibilities

• Manage the client intake process from initial inquiry through enrollment and onboarding.
• Conduct intake screenings, assessments, and interviews with prospective clients and families.
• Maintain accurate and up-to-date client records, documentation, and intake forms.
• Coordinate with internal care teams to ensure smooth client transitions and service placement.
• Respond promptly to phone, email, and in-person inquiries with professionalism and empathy.
• Track intake data and generate reports to support organizational goals; ensure compliance with regulations and confidentiality standards.
