Job summary

Arcare seeks a Specialty Patient Care Coordinator – Infectious Disease (SPCC-ID) to ensure HIV/AIDS patients and those with other infectious diseases receive coordinated, patient-centered care across their healthcare journey.

The SPCC-ID collaborates with patients, families, providers, and community partners to develop individualized care plans, coordinate services, and connect patients to medical, behavioral health, and community resources while maintaining Ryan White Program compliance.

Qualifications

• Bachelor’s degree in social work or related field required.
• 2–4 years of related healthcare coordination experience preferred.
• Experience with HIV/AIDS or infectious disease populations strongly preferred.
• TB test and CPR certification required; HIV/AIDS Non-Medical Case Manager training would be beneficial.

Responsibilities

• Client Assessment and Care Planning: conduct screenings, intakes, and re-screenings per Ryan White documentation.
• Care Coordination and Communication: serve as primary contact, coordinate across care settings, facilitate communication with patients and providers.
• Referrals and Resource Management: coordinate referrals to specialists and connect patients with financial assistance, community resources, and support services.
• Documentation and Data Management: maintain patient records and ensure regulatory/compliance requirements; use EHR systems.
• Outreach and Community Engagement: conduct outreach, provide culturally and linguistically appropriate services, build community partnerships.
• Training and Development: participate in ongoing training on care coordination, behavioral health, and resources.
