Signify Health, part of CVS Health, is seeking a Part-Time Clinician (NP or PA) to provide in-home health evaluations. You will engage with members in their homes, build trusted relationships, and help bridge gaps in care.

Candidates must hold active licenses, board certification, and be willing to travel locally within licensed states. This role offers flexible hours and a competitive pay range with benefits for part-time staff.

Qualifications

• Active, unrestricted licenses in the coverage area(s).
• Board certification is required.
• Multi-state licenses are preferred.

Responsabilités

• Be the in-home clinician visiting members in their homes across assigned geographies.
• Conduct 45-minute visits including review of medications and medical history, physical evaluation, and prescribed tests as needed.
• Collaborate with Signify Health clinicians to coordinate care and ensure quality standards.
• Travel locally and within licensed states as required; conduct virtual visits when needed.
• Perform diagnostic and preventive services as indicated (e.g., spirometry, bone density screening).
• Maintain documentation and use the clinical workflow on an iPad for each evaluation.
