Job summary

Signify Health, part of CVS Health®, is seeking a Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. You will visit members in their homes, review medications and medical history, perform a clinical evaluation, and offer additional tests as ordered by the health plan.

Travel locally and to licensed states is required. The role supports flexible scheduling, with 29.5 hours per week and a pay range of $41.85–$90.13 hourly.

Qualifications

• Active, unrestricted licenses in coverage area(s).
• Board certification required.
• Multi-state licenses preferred.

Responsibilities

• Visit members in their homes for In-Home Health Evaluations.
• Perform medication and medical history review, physical evaluation, and prescribed tests as ordered by health plan.
• Use iPad with clinical workflow; no prescribing or altering current regimens.
• Conduct virtual visits when needed.
• Coordinate with Signify Health services including diagnostic and preventive screenings.
