Job summary

At Home Hospice is seeking a Home and Community Based Services Aide to support patients in their homes within a 50-mile radius. This PRN/inconsistent position involves in-home hospice visits and helping clients maintain independence while providing compassionate care.

Responsibilities include personal care, daily living tasks, and documenting patient conditions using EVV. TX state requirements apply; CPR and 6+ months of home care experience are preferred, with flexibility to work varied hours

Qualifications

• Must have current Texas driver’s license, vehicle insurance, and reliable transportation or access to public transportation.
• Preferred: CPR certification or ability to complete within 90 days of hire.
• Preferred: 6+ months of home care experience.
• Preferred: ability to work flexible hours and independently.

Responsibilities

• Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, medication reminders).
• Support household tasks, meal preparation, and accompany clients to appointments or errands.
• Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system.
• Ensure a safe environment and operate medical equipment; respond promptly to client needs.
