Job summary

Heritage Home Health & Hospice is seeking a Chaplain to support patients, families, and the healthcare team. The role focuses on spiritual care, coordination with nurses, social workers, and therapists, and adherence to agency policies.

Responsibilities include serving as the spiritual liaison across patient care, and reporting to the administrator with regular meetings. CPR BLS certification is required; ecclesiastical endorsement is mandatory.

Qualifications

• One year of home health and hospice experience preferred but not required.
• Knowledge and skill with computers, fax machines, and healthcare systems.
• General knowledge of computers and ability to learn industry-specific software and documentation applications.
• Current CPR BLS certification.
• Accreditation by a chaplain organization preferred but not required.
• Ecclesiastical endorsement required.

Responsibilities

• Chaplains work with nurses, social workers, and therapists to provide patients, families, and other healthcare professionals with needed care and information.
• The Chaplain is a point of contact for all spiritual needs in patient care.
• The Chaplain follows agency, state, and federal policies and procedures.
• The Chaplain reports to the administrator and attends meetings as required.
