Job summary

Signify Health, part of CVS Health®, is seeking a Part Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

The role involves visits lasting about 45 minutes, using an iPad for the clinical workflow, and may include travel locally and within licensed states. No prescriptions or lab orders are required during visits.

Qualifications

• Active, unrestricted medical license in the coverage area.
• Multi-state licenses preferred.

Responsibilities

• Conduct in-home health evaluations for health plan members.
• Engage with patients, reviewing meds and medical history during visits.
• Travel locally and within licensed states as needed; conduct virtual visits when required.
• Provide Diagnostic & Preventive Services per Signify Health programs.
