Job summary

Kaiser Permanente in Vancouver, Washington seeks a Home Health Aide to deliver compassionate personal care and light household tasks under a physician-directed plan of care to support rehabilitation and independence.

You will assist with bathing, grooming, toileting, mobility, and medication reminders, document care, and report changes promptly. Requires CNA experience and WA/OR nursing assistant certifications; background check required.

Qualifications

• Minimum one year of CNA experience.
• High School Diploma or GED required.
• Nursing Assistant Certification - WA within 6 months of hire.
• Nursing Assistant Certification - OR within 6 months of hire.
• Must pass background check and drug screen.

Responsibilities

• Follow care plan from PHN or therapist to support ADLs and mobility.
• Provide bathing, grooming, toileting, and skincare.
• Assist with transfers, ambulation, and use of lifts as directed.
• Assist with medication reminders and patient safety checks.
• Document care provided and report changes promptly to supervisor.
• Perform light household tasks essential to care and safety.
• Attend required trainings and meetings and adhere to guidelines.
