Job summary

CenterWell Home Health in Washington is seeking a compassionate Home Health Aide to support patients at home, reporting to the Associate Director, Branch Administration. You will help patients regain strength, mobility and independence through rehabilitation-focused care.

This role requires CNA/CHHA credential, state certification, a valid driver’s license, and at least one year of HHA/CNA experience in hospital, nursing facility, or home health.

Qualifications

• Complete CNA or CHHA program within the last 24 months.
• Meet applicable state certification requirements.
• Valid driver’s license and reliable transportation.
• Minimum one year experience as HHA or CNA in relevant settings.

Responsibilities

• Provide direct patient care under RN and according to the Plan of Care.
• Assist with medications and monitor intake and vital signs.
• Assist with hygiene, bathing, grooming and nutrition.
• Support mobility, transfers and safety per Plan of Care.
• Document care and communications with the care team.
