Qualifications

• CNA, HHA, or equivalent home-care experience.
• Experience with fall-risk and mobility-impaired clients.
• Comfort with personal care tasks.
• Reliable, patient and attentive to individual needs.
• Ability to work in private home setting and follow care plan.
• Strong oral and written communication skills.
• Proficient using tablets or computers for documentation.
• Knowledge of care protocols and safety procedures.

Responsibilities

• Assist with mobility and transfers to prevent falls.
• Provide basic personal care (hygiene, toileting, dressing).
• Monitor and assist with oxygen therapy as needed.
• Prepare light snacks or drinks as requested.
• Offer companionship and emotional reassurance during shifts.
• Assist in positioning and turning to prevent discomfort.
• Document and report changes in client condition promptly.
• Respond to emergencies following proper procedures.
• Follow the patient’s plan of care and care policies.
