• Assist with activities of daily living and personal care including ambulation, bathing, dressing, eating, and personal hygiene.
• Provide companionship and social interaction to promote mental well-being and engagement.
• Assist with meal planning, preparation, light housekeeping, laundry, and errands per the care plan.
• Ensure client safety by monitoring home environment and reporting unsafe conditions to supervisor.
• Escort clients to medical facilities, appointments, and community activities as specified in the care plan.
• Follow written care plans and policies, documenting care provided and any changes.
• Coordinate with Branch Manager and Client Care Team to optimize service delivery.
• Support families with respite care and emotional reassurance as needed.
