Job summary

Qualifications

• Provide in-home personal care and assistance with daily living activities.
• Assist with activities of daily living, housecleaning, meal preparation, transportation, companionship, and respite.
• Ensure safety and adherence to agency policies and industry standards.
• Follow care plans and document observations for supervisor review.
• Support families with respite and emotional reassurance as part of the care team.

Responsibilities

• Assist with activities of daily living including ambulation, bathing, dressing, grooming, and mobility.
• Provide meal planning and preparation, light housekeeping, laundry, and shopping/errands.
• Offer companionship and encouragement of activities that stimulate the mind.
• Escort clients to appointments and outings as specified in the care plan.
• Document observations and report safety concerns to the Branch Manager.
• Follow the written care plan and participate in the Client Care Team discussions.
