Job summary

Live Well Home Care is seeking caregivers to provide in-home personal care to clients in their own homes and communities. Responsibilities include assisting with daily living activities, providing companionship, meal preparation, transportation, and respite care as outlined in care plans.

The role emphasizes compassionate, respectful service, adherence to agency policies, and collaboration with the Client Care Team to ensure client safety and well-being.

Qualifications

• Must have Physical and TB test results within last 11 months.
• Ability to assist with activities of daily living and personal care.
• Strong communication and compassionate nature.
• Reliability and adherence to agency policies and procedures.

Responsibilities

• Assist with activities of daily living and personal care (ambulation, bathing, dressing, etc.).
• Provide companionship and social interaction to clients.
• Perform light housework and meal planning as outlined in the care plan.
• Escort clients to appointments, errands, and social activities as needed.
• Observe client safety and report concerns to Branch Manager.
• Follow the written care plan and monitor supplies; participate in care team discussions.
