Job summary

Live Well Home Care is hiring for an in-home care role providing personal support to clients in their own homes and communities. You will assist with daily living activities, household tasks, meal planning, transportation, companionship, and respite care according to care plans.

Responsibilities include ensuring safety, coordinating with the Care Team, and documenting observations. The role emphasizes compassionate, patient-centered care and adherence to agency policies and standards.

• Ensure client’s safety by supervising the home environment
• Teach/perform meal planning and preparation, routine housekeeping (beds, dusting, vacuuming, washing floors, cleaning kitchen and bathroom, laundry)
• Provide companionship including social interactions, conversations, emotional reassurance, and encouragement of mind-stimulating activities
• Provides respite care for families per care plans
• Perform/assist with essential shopping/errands, may handle client money under the care plan and Branch Manager supervision
• Assist clients with following a written, special diet plan and reinforcement of diet maintenance as identified on the care plan
• Escort clients to medical facilities, errands, shopping, and outings as specified in the care plan
• Assist clients with writing or typing correspondence
• Participate with Client Care Team by providing input and suggestions
• Ensure service is delivered in accordance with policies, procedures, and practices
• Monitor supplies and resources
• Follow the written care plan
• Carry out duties as assigned by the Branch Manager
• Observe clients and their environments and report unsafe conditions to Branch Manager
