Job summary

Lawndale Christian Health Center seeks a Home Care Aide to assist participants with daily living tasks and personal care in their homes. You will help with meals, housekeeping, shopping, and transportation as needed, following the Plan of Care.

The role requires travel across the Greater Chicagoland area to provide high-quality in-home services while maintaining client confidentiality and documentation in accordance with CCP guidelines.

Qualifications

Responsibilities

• Assist with household tasks and personal care per Plan of Care.
• Travel throughout Chicagoland to deliver in-home services.
• Maintain confidentiality of participant information.
• Document daily activities, observations, and hours of service.
• Attend trainings and staff meetings as scheduled.
• Provide escort/transportation for medical visits and errands as needed.
• Follow safety procedures and report emergencies per protocol.
