Qualifications

Responsibilities

• Implements and provides ADL/IADL support to clients at home as ordered.
• Assist with personal care including baths, grooming, dressing, toileting.
• Assist with transfers, ambulation, and therapy exercises under a therapist direction.
• Maintain a clean and safe home environment and assist with meals according to care plan.
• Provide companionship and accompany clients to appointments and activities.
• Document shift details and ensure HIPAA compliance.
