Job summary

Acts Hospice, part of Acts Retirement-Life Communities, Inc., is seeking a Social Worker to deliver and document individualized social work assessments, counseling, and case management for hospice participants and families as directed by the plan of care.

The role supports end-of-life planning, coping with grief, and collaborates with the Interdisciplinary Team (IDT) to ensure compassionate, person-centered care.

Qualifications

• Graduate degree in social work or related field.
• Current licensure in the state where services are rendered.
• Minimum of two years' experience in hospice or a transferable, clinical venue of social work practice such as oncology, geriatrics, or community health.

Responsibilities

• Developing, providing, documenting, and evaluating individualized social work assessment, counseling, and casework services to hospice participants and families as directed by the plan of care.
• Counsels hospice participants and families on issues related to self-determined life closure, safe and comfortable dying, and effective grieving, in collaboration with the Interdisciplinary Team (IDT).
