Job summary

Health Care Connectors is seeking a qualified and compassionate Social Worker to join our team in a home healthcare setting. The ideal candidate will have experience with hospice, home health, dementia care, and developmental disabilities, and will provide care and support to patients and families while developing systems to improve outcomes.

Responsibilities include assessing psychosocial needs, creating care plans, counseling patients and families, and collaborating with healthcare

Qualifications

Must hold a Master’s degree in Social Work or related field.
Experience working with adults and elderly populations.
LCSW or LMSW license required.
Driver's license and car insurance.
Strong communication skills and teamwork ability.
Ability to work independently as well as part of a team.

Responsibilities

Assess psychosocial status of patients and families and relay findings to the care team.
Assess patients' needs and develop care plans.
Provide individual and group counseling to patients and families.
Collaborate with healthcare professionals to ensure quality care.
Offer information and referrals to patients and families.
Serve as liaison between patients, families, and community agencies.
Develop systems to improve patient outcomes.
Conduct research to identify best social work practices.
Educate and train staff on social work topics.
Maintain accurate records of patient care.
