Qualifications

• MSW degree from an accredited program.
• Current state social work license is required.
• 2+ years of clinical social work experience, preferably in healthcare or hospice.
• Reliable transportation and ability to lift up to 25 lbs.

Responsibilities

• Conduct psychosocial evaluations of patients and families.
• Provide ongoing counseling to patients and families during their end-of-life journey.
• Support emotional well-being aligned with the plan of care.
• Collaborate with the leadership team and care staff to ensure compassionate care.
