Job summary

Masonic Villages of Pennsylvania - Home Health, Hospice, and Home Care Campus is hiring a Hospice Social Worker on a part-time, daytime schedule. The role focuses on assisting patients, families, and caregivers with emotional and practical issues at end of life to improve quality of life.

The ideal candidate holds a four-year degree in social work with MSW being preferred, and has experience in hospice social work and healthcare settings. A PA driver’s license is required.

Qualifications

• Bachelor's degree in social work is required.
• MSW is required by Medicare Conditions of Participation.
• Personal computer experience is required.

Responsibilities

• Integrate social services within hospice care.
• Perform psychosocial assessments for patients and families.
• Assist with advance care planning and bereavement support.
• Coordinate with hospice team and external resources.
• Document per Medicare regulations and hospice standards.
• Participate in on-call supportive services rotation.
