Job summary

Masonic Villages of Pennsylvania - Home Health, Hospice, and Home Care Campus is hiring a Hospice Social Worker on a part-time, daytime schedule. The role focuses on assisting patients, families, and caregivers with emotional and practical issues at end of life to improve quality of life.

The ideal candidate holds a four-year degree in social work with MSW being preferred, and has experience in hospice social work and healthcare settings. A PA driver’s license is required.

Qualifications

• MSW is required by Medicare Conditions of Participation.
• Personal computer experience is required.

What You'll Do as a Hospice Social Worker:

As a Hospice Social Worker, you will carry out our Mission of Love by assisting patients, families and caregivers with emotional and practical issues at the end of life with the goal of enhancing quality of life.

Our Hospice Social Worker is responsible for the following:

• Assures that social service activities are integrated and coordinated within the hospice.


• Develops specific, measurable, realistic plans and objectives with the goal of quality of life and safety in mind.


• Performs psychosocial and bereavement assessments on all patients and families.


• Serves as a resource to patients and families for questions and emotional support. Identifies and addresses psychosocial needs of patients and families.


• Demonstrates knowledge of clinical social work.


• Communicates and collaborates appropriately and clearly to all members of the hospice and facility/referral source teams.


• Facilitates admission meetings and demonstrates competency and proficiency in explaining Medicare hospice regulations, advanced care planning, and hospice education for patients, families, and referral sources.


• Demonstrates the ability to be flexible, organized and function under stressful situations.


• Makes referrals to outside agencies or groups as needed.


• Demonstrates cultural competency in work.


• Documents in accordance with Medicare regulations and hospice department standards.


• Is an active member of the team meetings.


• Participates in the on-call supportive services rotation.


• Participates in committees and projects as appropriate.


• Carry out QAPI roles and responsibilities as assigned.

What You Need as a Hospice Social Worker:

• Graduation from a four-year college or university with a degree in social work.


• MSW required in accordance with Medicare Conditions of Participation


• Personal computer experience required.


• Hospice or Medical Social Work experience preferred.


• Maintains a current PA driver' s license and insurance on personal vehicle.
