Responsibilities

• Provides professional counseling and psychosocial assessments for patients and families.
• Assesses financial needs and explains Medicare/Medicaid and insurance benefits.
• Performs crisis intervention and suicide risk assessments.
• Participates in care planning with the interdisciplinary team and updates plans of care.
• Documents services in the medical record and coordinates community resources.
• Recruits, trains, and supervises hospice/home care volunteers.
