Job summary

Qualifications

• Must hold a social work degree from an accredited program.
• Licensed LMSW in the state and current certification.
• Knowledge of local community resources and patient advocacy.

Responsibilities

• Provide coordinated in-home psychosocial care for patients of all ages.
• Perform psychosocial assessments, counseling, and referrals.
• Complete Comprehensive Assessment within five days of hospice election.
• Assist with advance directives, discharge planning, and family counseling.
• Participate in IDG/IDT meetings and care planning.
• Promote hospice philosophy and administrative policies.
• Engage in QAPI activities and maintain patient/privacy standards.
