Job summary

Hospice Home Care in Hot Springs, AR seeks a compassionate Social Worker to provide psychosocial support to patients and families. You will assess needs, counsel, coordinate resources, and collaborate with the care team to deliver holistic hospice services.

The role emphasizes education on advance directives, documentation, and liaison with community resources. A MSW or BS in Social Work and state licensure are required, with hospice experience preferred.

Qualifications

• MSW or BSW accredited by the Council on Social Work Education.
• Current licensure to practice in state of operation.
• Minimum of one year of health care experience; hospice experience preferred.
• Strong verbal and written communication and organization skills.
• Must have a licensed driver with insured automobile.

Responsibilities

• Assess psychosocial needs of patients and families.
• Evaluate home safety; assist with alternative care plans.
• Provide counseling, crisis intervention, and emotional support.
• Maintain accurate and timely clinical documentation.
• Educate and assist with advance directives.
• Provide referrals and practical support for patients and families.
• Serve as liaison with community resources and agencies.
• Collaborate with team to support holistic patient care.
• Support funeral, financial, and legal planning with families.
