Qualifications

• MSW CSWE- or BSWSWE-accredited social work degree required.
• Minimum 1 year health-care experience; hospice preferred.
• Current state licensure to practice social work; valid driver license with insured vehicle.

Responsibilities

• Provide counseling, crisis intervention, and emotional support.
• Educate and assist with advance directives and practical planning.
