Job summary

Qualifications

• Must hold a Masters of Social Work from an accredited program.
• At least one year of social work experience in a health care setting.
• Licensed Social Worker (LSW/LMSW) or equivalent per state requirements.

Responsibilities

• Provide emotional and logistical support to patients and families at end of life.
• Assist with end-of-life planning, finances, funerals, and related arrangements.
• Collaborate with interdisciplinary team to coordinate care and services.
• Provide bereavement support and grief counseling to families.
