Job summary

VITAS Healthcare, the nation’s leading end-of-life care provider, seeks a hospice social worker to deliver emotional and logistical support to patients and their loved ones in home settings throughout California.

The role requires an MSW and at least one year of health care social work experience, with preference for experience with terminally ill patients and families. You will collaborate with RNs, physicians, chaplains, and the IDG to extend comfort and dignity at the end of life.

Qualifications

  • MSW required from CSWE-accredited program.
  • At least one year of health care social work experience.
  • Experience with terminally ill patients and families preferred.
  • Experience in providing counseling and support to patients and families.
  • Empathy and compassion in dealing with seriously ill patients and their families.
  • Equivalent licensure considerations per state rules (license/certification may be required).

Responsibilities

  • Provide emotional and logistical support to patients and families.
  • Assist with end-of-life planning including finances, funeral arrangements, and personal wishes.
  • Offer bereavement support and grief counseling.
  • Collaborate with the interdisciplinary team to deliver 360-degree care.
  • Travel to patients' homes as required by the job.
  • Educate patients and families on emotional responses to illness progression.
