Job summary

Luminos Hospice is seeking a Hospice Scheduler to coordinate daily and weekly schedules for nurses, social workers, and volunteers, ensuring coverage based on patient needs.

Qualifications

• Healthcare setting experience required or preferred.
• LVN/LPN strongly preferred as part of requirements.

Responsibilities

• Develop and maintain daily and weekly schedules for hospice staff, including nurses, social workers, and volunteers.
• Coordinate shifts and on-call assignments to ensure coverage based on patient needs.
• Serve as the primary point of contact for scheduling inquiries and updates.
• Communicate effectively with patients, families, and healthcare providers about scheduled visits and any changes.
• Maintain accurate and up-to-date records in the scheduling database.
• Ensure compliance with regulatory guidelines related to scheduling and patient care.
• Address scheduling conflicts proactively and implement solutions.
• Respond to urgent scheduling requests and adjust schedules as necessary.
• Prepare and present scheduling reports to management, highlighting trends and areas for improvement.
