Job summary

Prisma Health is seeking a Registered Nurse for Home Health - Midlands, to assess, develop, implement and evaluate nursing care for assigned patients. The role coordinates with other disciplines within the Home Nursing department to ensure comprehensive care.

Responsibilities include ongoing assessments, discharge planning, and documentation per agency policy, with required licensure, BLS, and a valid driver’s license.

Qualifications

• Education - Associate degree in Nursing
• Experience - One (1) year of experience as a Registered Nurse
• Licenses - RN license recognized in state of work and BLS
• Driver requirements - Valid driver's license and auto insurance

Responsibilities

• Promotes and documents communication with other disciplines regarding patient care and agency function.
• Performs assessments and interventions at appropriate intervals; observes progress and adjusts care accordingly.
• Initiates discharge planning to address ongoing patient needs.
• Submits paperwork and participates in audits and quality improvement projects.
• Follows agency policies and procedures and maintains confidentiality.
• Performs other duties as assigned.
