Job summary

St. Croix Hospice in Kansas is seeking a Registered Nurse Case Manager to deliver personalized hospice care in patients' homes.

You will collaborate with a care team to create a plan of care, lead compassionate end-of-life conversations, and manage symptoms with clinical excellence. This role emphasizes autonomy, patient connection, and thorough assessments, with on-call duties and a standard schedule from 8:00am to 5:00pm.

Qualifications

• 1+ years of acute care experience required; hospice/home health experience preferred
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
• Must have reliable transportation, valid driver's license, and proof of auto insurance
