Job summary

IntelliResume in East Hartford, CT seeks a Hospice Weekend Registered Nurse to provide skilled home-based end-of-life care and support families.

You will assess, plan, and implement care with the interdisciplinary team, supervise LPNs and home care aides, and arrange referrals.

Part-time, day shift; requires an associate degree or nursing diploma; NLN BSN preferred; minimum one year of nursing experience in medical-surgical or home care.

Qualifications

• Graduate of an Associate’s or nursing diploma program.
• NLN approved BSN program preferred.
• One year nursing experience OR one year nursing experience in a specialty; home care or medical-surgical experience is common.

Responsibilities

• Provide skilled nursing care to hospice clients in homes or community settings.
• Review referrals and initiate contact with clients and families.
• Conduct complete nursing assessments based on referral data and clinical observations.
• Develop a plan of care in collaboration with clients and all disciplines.
• Facilitate peaceful death and assure appropriate bereavement follow-up.
• Modify plan of care as conditions change.
• Utilize interdisciplinary team members to support patient/family needs.
• Teach patients and reinforce client/caregiver education.
• Direct LPNs and Home Care Aides in carrying out the established plan of care.
• Refer clients/families to other community resources as needed.

Education
