Job summary

AccentCare, Inc. is seeking a Hospice Registered Nurse (RN) to join our care team in Connecticut. You will provide compassionate, patient-centered nursing care within an interdisciplinary group, focusing on comfort and quality of life for hospice patients and their families.

This role offers autonomy to practice at the top of your license while supported by a mission-driven organization that values well-being and professional growth. Minimum one year of nursing experience and RN license required.

Qualifications

• RN license in the state of practice.
• At least one year of nursing experience.
• Reliable transportation and valid driver’s license.

Responsibilities

• Provide individualized nursing care that maximizes comfort and quality of life.
• Partner with patients, families, physicians, and the interdisciplinary group to create and carry out care plans.
• Perform comprehensive assessments to guide care decisions.
• Educate and support patients and caregivers, meeting them where they are.
• Coordinate care across disciplines to ensure continuity and dignity.
• Document care clearly and accurately reflecting patient needs and progress.
• Participate in on-call or admission rotations as needed.
• Engage in ongoing learning and professional development.
